What Are Your Cancellations Really Costing Your Clinic?

If you're a pediatric clinic owner, you've probably accepted cancellations as part of the job.

Kids get sick.

Families have emergencies.

Schedules change.

Some cancellations are unavoidable.

But what if your cancellation rate is telling you something bigger?

What if it's not just costing you REVENUE?

What if it's costing your therapists, your families, and the long-term health of your clinic?


The Numbers Are Bigger Than Most Clinic Owners Realize

Most clinic owners know cancellations hurt.

Few actually calculate the cost.

Let's look at a simple example.

Imagine you have:

  • 15 therapists

  • Each therapist sees six clients a day

  • Your average reimbursement is $120 per visit

  • Your cancellation rate is 20%

That works out to more than $500,000 a year in lost revenue.

Even at a 15% cancellation rate, you're still losing hundreds of thousands of dollars annually.

That's money that could be invested back into your clinic.

Better salaries.

New equipment.

Continuing education.

GROWING YOUR TEAM.

Instead, it disappears one missed appointment at a time.


The Real Cost Goes Beyond Revenue

It's easy to think cancellations are just a scheduling problem.

They're not.

Every cancellation creates a ripple effect throughout your clinic.

Therapists lose momentum with their clients.

Children make slower progress.

Waitlists grow longer.

Families who are ready for services continue waiting.

YOUR SCHEDULE BECOMES UNPREDICTABLE.

And over time, therapists begin feeling frustrated.

Not because they're working hard.

Because they aren't seeing the progress they know is possible.


I Stopped Asking, "Why Are Parents Cancelling?"

For a long time, I accepted cancellations as part of pediatric therapy. Then I decided to dig deeper.

As I shared in an earlier post, going back for my DOCTORATE lled me to research parent engagement — and that research is exactly why I stopped treating cancellations as a scheduling issue.

It wasn’t simply about attendance. It was about parent engagement. The more connected parents felt to the therapy process, the more likely they were to attend consistently, participate during sessions, and follow through at home.

That changed everything.


Parent Engagement Is a System, Not a Personality Trait

Many therapists assume some parents are simply more engaged than others.

I don't believe that's true.

Parents respond TO THE SYSTEMS WE BUILD.

If we don't clearly explain their role...

If we don't coach them during sessions...

If we wait until the last five minutes to talk with them...

We shouldn't be surprised when they stay in the waiting room or skip appointments.

Engagement doesn't happen by accident.

It happens by design.


Your Therapists Need More Than Good Intentions

One of the biggest changes we've made in our clinics wasn't creating another policy.

It was giving therapists better language.

Many therapists worry they'll upset parents.

They avoid difficult conversations because they don't want to create conflict.

But when those conversations don't happen, attendance suffers.

Discharge conversations get delayed.

Parents stay confused about their role.

And therapists carry the weight of trying to do everything themselves.

CONFIDENCE isn’t something therapists automatically have.

It's something we coach.


Better Parent Engagement Reduced Burnout Too

One unexpected outcome surprised me.

As parent engagement improved, therapist burnout started to decrease — because therapists finally had partners in the room instead of carrying every session alone.

Parents helped with transitions. They supported EMOTIONAL REGULATION.. They learned how to respond during difficult moments instead of watching from the sidelines.

And when therapists feel supported instead of stretched thin, they’re more consistent with families, which means fewer last-minute cancellations and more families staying engaged for the long run.


Stop Accepting High Cancellations as "Just Pediatrics"

Yes, children will get sick.

Life will happen.

Some cancellations are unavoidable.

But if high cancellation rates have become your normal, it's worth asking a different question.

What inside my clinic is contributing to this?

Because cancellations rarely exist in isolation.

They're often connected to communication.

Expectations.

Systems.

Parent engagement.

And leadership.

When you improve those areas, attendance often improves too.
